What is HCP?
The Home Care Package (HCP), was established by the Australian Government to assist older Australian's in living independently, whilst accessing care services.

The program aids in delivering flexible and tailored support according to an individuals health care needs.
​
Funding is allocated to help a person stay well and independent, comfortable at home and connected to their community.

How does it work?
Individuals can check their eligibility via myagedcare.com.au or by calling 1800 200 422.
​
If eligible, an assessor from the Aged Care Assessment Team will determine suitability for funding.
​
Once assessed, individuals who meet the criteria will be assigned a package which is deemed appropriate for health care needs.
​
Our friendly On The Road Care team can assist throughout this entire process, ensuring you or your loved one receive a package fit for your support needs and lifestyle.

Who is eligible?
To recieve funding, individuals need to be aged 65 or older, (50 years or older for Aboriginal and Torres Strait Islander peoples), and have care needs or conditions determined to be age-related or complex.

Funding is allocated under four levels of care.
​
Level 1 = Basic Care
Level 2 = Low Care
Level 3 = Intermediate Care
Level 4 = High/ Complex Care
​
Individuals already receiving funds under other Government schemes or programs may not be eligible.
